The Applicant agrees that the data shared with Uzm. Dr. Kamile Demirci (www.kamiledemirci.com) is correct, will update this data in case of any changes, and accepts and declares that our clinic is not responsible for any damages that may arise as a result of not updating this data or providing incorrect data.

The Applicant expressly consents to the collection, recording, processing, storage, use and transfer/sharing of the personal data shared with our clinic by our clinic for the purpose of performing appointment procedures, providing medical services, conducting treatment processes, fulfilling our legal obligations and creating a source for future services. The third parties to whom the Applicant authorizes the transfer and sharing of personal data are, without limitation, as follows:

Domestic and/or foreign third parties with whom our clinic has a service relationship, research, promotion and consultancy companies that process data on behalf of our clinic. This information will only be shared with third parties who act in accordance with the relevant legislation on data protection and security of our clinic, only in case of need and to the extent necessary, in order to fulfill the rights and obligations of the parties regarding the medical service relationship to be established with the Applicant.

Rights under the Personal Data Protection Law (KVKK)
Pursuant to Article 11 of the Law on the Protection of Personal Data, the Applicant may request the following rights by making an application with wet signature to the address of our clinic Yenişehir Mah. Demokrasi Bulvarı No:27/3 KOCAELİ/İZMİT:

Learn whether their personal data is being processed,

Request information if their personal data has been processed,

To learn the purpose of processing personal data and whether they are used in accordance with their purpose,

To know the third parties to whom personal data are transferred domestically or abroad,

To request correction of personal data in case of incomplete or incorrect processing,

To request the deletion or destruction of personal data within the framework of Article 7 of the KVKK,

To request notification of the correction, deletion, destruction of personal data to third parties to whom personal data are transferred,

To object to the emergence of a result to the detriment of the person himself/herself by analyzing the processed data exclusively through automated systems,

In case of damage due to unlawful processing of personal data, to demand the compensation of the damage.
